It's great that you want to join The Blue Toes. Our new courses start in the third week of September 2024. By filling in this form you're applying to become a member. The secretary will process your application. You will hear from us as soon as possible. You can only become a member in the current year if you can buy an ACLO year or semester card. Contribution for The Blue Toes is €40,- per year, this will stay the same for the coming year. Paying contribution is mandatory. The dance year runs until the 31st of August. You have to unsubscribe before the 1st of July of the ongoing dance year. Choose one of the dance courses shown below.

With this form, we want to ask permission for processing your information in several ways within the association. You can give us permission by checking the boxes and deny it by leaving the box unchecked. In some cases you cannot check a box, this means that you have to agree with this condition to become a member. Of course, it is possible to make any combination of checked and unchecked boxes. If your preferences for processing your information change, you can always let us know by sending an email to secretaris@bluetoes.nl, specifically telling us which parts you would like to change.

Activities

Sometimes we need to spread your information among the organizers of a certain event (such as the address of a cook for a running dinner) or other participants of an event (in case you have to prepare something together). For each activity, it will be clear who gets which information and whether or not information has to be spread at all.

Photo's

At our activities a photo committee will be present. It is possible that they will take a picture of you.

Committees

If you want to be a member of a committee, there are a few more ways in which we would like to process your information. In case you are not (yet) in a committee, we still ask you to fill in this part.

How did you get to know us?

Signing

By filling in and signing this registration form, I declare to become a member of SDV The Blue Toes until I resign. During my membership, I will conform to the statutes and the internal regulations of The Blue Toes. If I haven’t taken note of these, I will do so as soon as possible. You can find the legislative acts and household rules here https://bluetoes.nl/gallery/files/?path=Documenten%2FHR. Or when I am a member, I can login and find them on the website of the association: http://www.bluetoes.nl/, under ‘for members’ -> ‘documents’. I own a valid ACLO Card. If I do not have a valid ACLO Card, I will be obligated to pay a fine of €20,00. I will pay all extra costs for failing to obtain a valid ACLO Card in time. I am aware that I will have to renew my sports card every dance season. The dance season runs parallel with the academic year.

I grant SDV The Blue Toes permission to withdraw money from my bank account for the following:

• The fee for the courses that I am taking (see above at courses) and the annual contribution.
• Additional costs for failing to acquire an ACLO Card (in time).
• Signing up and participating in Blue Toes activities (including NTDS and ETDS).
• Purchase of association clothing or attributes on my request.

By signing, I declare that I have correctly and completely filled out this form. I will notify the secretary of The Blue Toes as soon as possible when personal data or course enrolments change.

When I want to terminate my membership for the next dance season, I will do this before July 1st of the ongoing season by sending an email to the secretary of The Blue Toes (secretaris@bluetoes.nl).

My e-mail address will be used to send me necessary information about withdrawals and the state of my membership. Add the following e-mail addresses to your contact list to avoid missing our mails: bestuur@bluetoes.nl, voorzitter@bluetoes.nl, secretaris@bluetoes.nl, penningmeester@bluetoes.nl, intern@bluetoes.nl, extern@bluetoes.nl.

The association has an AVG-form where privacy preferences can be stated about personal information, registration for activities, photo’s in which I am recognizable, committees that I am a member of. My filled in AVG-form can always be requested from the secretary.

Recurrent SEPA direct debit mandate form*

By signing this form, I authorize until termination of my membership:

• SDV The Blue Toes to send instructions to my bank to debit my account for membership (contribution and any relevant ACLO fines), participation of courses (course fees), participation of activities and the purchasing of association clothing or attributes.
• My bank to debit my account in accordance with the instructions from SDV The Blue Toes.

As part of my rights, if I don’t agree, I am entitled to a refund from my bank under the terms of conditions of my agreement with my bank. A refund must be claimed within eight weeks starting from the date on which my account was debited.

You will receive the confirmation of your registration by email when your registration has been accepted by the secretary. If the secretary has any questions regarding your registration, the secretary will contact you.
